prp ADULT eligibility
Ark of Rehabilitation Inc. PRP Program services to adult ages 18 and up, who have been diagnosis with a mental health disorder and are actively working with a therapist and/or psychiatrist.
- Referral from treating Licensed Mental Health Professional and Fax to 443-815-4746
- Maryland Medicaid and/or proof of application
- A psychiatric diagnosis that meets the priority population as determined by the Mental Health Authority
- Intake and Rehabilitation Assessment with AORI License Rehabilitation Specialist.
AORI ADULT PREPARENESS
At AORI, we help our Adults to be able to live independently.
it's our believe that every person should have a stable housing, Education, Financial Stability, Healthcare and PRP Counselor.
CLIENTS SUPPORT:
Stable Housing: Ark of Rehabilitation Inc. will assist Adults to secure stable living conditions/situations.
Financial Stability: Ark of Rehabilitation Inc. will assist with client financial resources, (i.e. to find and Apply for benefit) assist with employment or entitlements to allow for self-sufficiency; AORI will ensure our clients can turn on their utilities in their own home.
Education: Ark of Rehabilitation Inc. will assist Adults to obtain a High School Diploma or Ged in order to attend College or a Trade School.
Healthcare: Ark of Rehabilitation Inc. will connect adults to the appropriate healthcare services, and able to address physical and Medical needs.
PRP Counselor: Adult clients will have a PRP Counselor, that will meet with them six time a month to address any situations.
